When you make your initial appointment we will send a packet of information about our practice and a questionnaire. Please complete the information before your appointment to streamline your first visit with us.
Please allow 60-90 minutes for your initial visit. Bring as much information as you can regarding prior treatments, x-ray reports, laboratory tests, and insurance plans. During this time we will go over the reports with you, perform an examination, and schedule any needed testing. Our initial goal is to establish a relationship with you and to arrive at a treatment plan which addresses your needs. We will make it possible for you to begin treatment quite soon after your initial evaluation if you so desire.
Be aware that our practice often treats patients with serious and life-threatening conditions. At times it is necessary to devote time to an established patient's concerns on an urgent basis, even though they may not have a scheduled appointment. Although this may impact new patient scheduling times, we will make every effort to see you in a timely manner and be aware that if you will ever need to be seen on an urgent unscheduled basis we will work you into the day's schedule.
Mallory Williams is our administrative assistant and an integral member of our practice. She is committed to making your visit a very positive and efficient experience. She will schedule appointments and tests, interface with your insurance company, and complete many other functions essential to your care. Over the years, the amount of time spent on paperwork and forms has increased dramatically. If you need to have special forms completed there may be an administrative charge for the service.
